COMMUNITY HEALTHCARE FOUNDATION MISSION STATEMENT: The Community Healthcare Foundation pledges to carefully consider the disbursement of private gift support for the advancement of superior healthcare services and the provision of educational resources throughout all stages of life in the communities in which we serve. Have you ever wondered what the highest honor is that can be bestowed upon a health care provider? It’s the decision of a patient, family member, or community member to help the provider by means of a financial contribution. Crittenden Health Systems (CHS) is the community’s health system, providing the County Hospital, EMS services, 24-hour Emergency care, Marion Home Health, and a full service Rehabilitation Department. The CHS professional staff represents many specialties whose services cover a wide range of inpatient and outpatient surgical services, Diagnostic Imaging including 24-hour MRI and CT Scanning, Mammography, Ultrasound and Bone Density Scanning, Critical Care Unit, and state-of-the-art Echocardiogram diagnostic capabilities just to name a few. Your tax deductible contributions will help CHS continue to provide the best technological advances in medical care that are not found in most rural hospitals. Your support is vital to this mission and the future of our community health system. We Treat You Like Family The CHS Community Healthcare Foundation's mission is to ensure your philanthropic support will enhance the quality of life for our patients, their families and loved ones. The Foundation Board of Directors oversee the distribution of all donations to provide scholarships in health care fields, new technologies, new facilities and community based support of healthy living.
The Donor Recognition Program was designed to express our sincere appreciation to those individuals who have helped ensure the mission of Crittenden Health Systems by giving the ultimate gift… something they have acquired during their lifetime. Nine levels of giving have been established to recognize our supporters. Donors are placed into giving levels according to the total donations made during a calendar year. Levels of Giving:
Donors at the Companion through Philanthropist levels of giving are honored annually at a recognition dinner where they are also presented with an appreciation gift. The names of the donors who participate at these levels will also be placed on a plaque located on the organizational Wall of Honor soon to be located in the Crittenden County Hospital lobby. CHS acknowledges all donations in writing upon receipt. Acknowledgments will be made to both the donor and the honoree or designated family member. Donor names are also listed in the organizational and community newsletters, unless donors request otherwise. CHS is a not-for-profit 501(c)(3) corporation, which means donations are tax deductible.
