COVID-19 (Coronavirus) Vaccine

Note: We are currently vaccinating Phase 1a all Tiers and Phase 1b - Tier 1. We will not be taking walk-ins. We do not have a waitlist for any Phase or Tier not currently open for vaccination.

Crittenden Community Hospital has created a Moderna COVID-19 Vaccination Appointment site. On this site you can easily schedule yourself into open appointment slots. You will need to login into the site using an email address, Facebook or Google login. If you are scheduling more than (1) appointment, you may schedule up to (2) appointments using the same email address, Facebook or Google account.



Appointments are for 1st doses ONLY. For 2nd dose, you should have a date and timeframe on the back of your vaccine card. You may get a text closer to that date with a specific time, but as long as you arrive during the timeframe indicated on your card, you will be vaccinated.
If you do not have an email address, Facebook or Google account, please call 270-965-1014, leave your name, phone number and state you do not have an email address, Facebook or Google account, and we will contact you to schedule an appointment.
