Helping you plan your next steps.

Our Care Management team helps patients and families understand their care needs and connect with support during and after their visit to Crittenden Community Hospital.

As a Rural Emergency Hospital, we provide emergency, observation, and outpatient services. Our team works with patients, families, and healthcare providers to coordinate follow-up care and assist with transfers when inpatient hospitalization or additional services are needed.

How We Can Help
• Identify needs and help plan for a safe transition home or to another care setting.
• Coordinate referrals for home health, medical equipment, and community resources.
• Help connect patients with mental health, substance use treatment, hospice, and other support services.
• Communicate with healthcare providers and insurance representatives to assist with care coordination.
• Help patients and families understand recommended next steps and follow-up arrangements.

Planning Your Next Steps
Planning begins during your visit. We consider your care needs, available support, and any concerns about managing at home. Patients and family members are encouraged to ask questions and participate in planning.
